Please see the attached guide on how to register yourself. Please read Step 8 as I have needed to add one minor instruction to help make my life easier upon confirming your registration and your roles within the club. Especially important for anyone who coaches, manages, or referees.
As mentioned on Thursday during training, the club will be covering the cost for the Accidental Medical Reimbursement. Basically, any additional medical needs that arise from a single on-field injury may be claimed under this policy. Please see attached document for a breakdown of the more common needs associated with the Accidental Medical Reimbursement policy.
Furthermore, should you feel the need to be protected even further in case of paralysis, you may add that on as well to increase your coverage. HOWEVER, it is not included into your dues and must be completed separately. Please see me for more info or to add on another extended health coverage.
